Please have the following documents in hand in order to efficiently register your child at GMS: (*=Required)WELCOME, New Student Registration Requirements:
- Withdrawal form – ONLY if withdrawing during the school year.
- Last report card
- Birth certificate
- Social security card
- Immunization record is needed at time of enrollment. Please submit on Georgia form #3231 within 30 days. Ear, eye and dental on Georgia form #3300 is due within 120 days of enrollment.
- Proof of residency in the form of a current utility bill. An example would be a current Georgia Power bill.
- Custody / guardianship paper or court orders (if applicable).
- Standardized Test Data – Helpful in appropriate placement. Custody/Guardianship Papers (If Applicable)
